© 2021-2026 Southlake Christian Counseling | Good Faith Estimate

The “No Surprises Act of 2022” went into effect January 1, 2022 and requires all medical providers to give a Good Faith Estimate to new and current counseling clients who pay for services out of pocket. The hope of the new law is to limit billing surprises for patients. The services of Southlake Christian Counseling therapists are provided at $135.  Southlake Christian Counseling is owned and operated by Licensed Clinical Social Worker – Shannon Thomas, and her National Provider Identification number is #1124262373. All services are in-person at 1560 East Southlake Blvd #100 Southlake Texas 76092. Southlake Christian Counseling provides weekly outpatient mental health services to individual adults. The Good Faith Estimate is only that, an estimate of current and future session fees, along with the duration of each counseling session. Advance notice of any fee increases will be given to help clients plan for a session rate change. If there are any questions or concerns regarding session fee charges, please first speak with your therapist, and if not resolved to your satisfaction, please contact Ms. Thomas at 817-897-8882. Any expressed concerns with session fees will not affect the quality of the therapeutic services you are receiving.  You can learn more about the “No Surprises Act of 2022” at  www.cms.gov/nosurprises  The Good Faith Estimate is not a contract but simply a statement of our current fees and expected length of services as it relates to the medical care you are receiving at Southlake Christian Counseling.
